Q. Where are you located?

Q. Will my insurance cover the cost of care?

A. You are ultimately responsible for your bill and for verification of your plans' benefits.
All new patients are asked to fax or mail a copy of their insurance card, front and back, and the registration form so that our staff can pre-verify your benefits before your visit. This will considerably shorten your wait before your appointment.

Q. Will I be skin tested the same day as my initial appointment?

A. In most instances, skin testing is performed during the initial office consultation, if indicated. You will meet with Dr. Levy first to complete a history and physical exam. If testing is performed, allow up to 2 hours for the initial evaluation, testing and consultation.

  • Make certain you are not taking antihistamines for 2 days prior to your visit (5 days for Zyrtec, Allegra, Atarax and Hydroxygine).

  • If you are too uncomfortable to discontinue these medications, prior to your visit, please call our nursing staff to recommend a prescription alternative that can be taken prior to testing.

Q. Is skin testing painful?

A. For most patients, skin testing is of minimal discomfort. We use a multi-test device for young children that completes 8 tests at once and is well tolerated. For older children, and adults, the derma-pik device feels like a tiny bristle-brush and is by far the least painful skin test device on the market.

For children over 3 years of age, our nursing staff can call in a prescription cream, EMLA, to be applied to the skin at home before arrival to minimize discomfort. This is a cream formulation of Lidocaine, used by dentists to numb teeth before dental work and works well to minimize discomfort during testing. Adults who have concerns with discomfort during testing can also use EMLA cream.

Please call our nursing staff at least two days before your appointment to have this cream medication called in for you.

Q. What do I do if I get a respiratory infection between regularly scheduled visits?

A. Dr. Levy does not treat acute respiratory infections, including colds, viruses, sinusitis and bronchitis. Your will need to contact your primary care physician or ear, nose and throat physician to treat these infections.

Q. Am I required to obtain a referral from my primary care physician?

A. Each patient is responsible for knowing their insurance plan requirements and for obtaining the appropriate referral forms prior to their visit. Failure to obtain this written authorization will result in patients being responsible for the entire balance of all charges incurred during their visit.

Q. How do I determine if Dr. Levy is a provider on my plan?

A. Our practice participates with Medicaid, Medicare and most health care plans except for closed HMO panels such as Blue Choice HMO & Kaiser Permanente. Many plans have "sub-plans" or "networks" contracted by your employers. Always check with your insurance company to be certain we participate in your specific plan. Georgia law holds the patient responsible for charges incurred by non-participating physicians.

Q. How long do office visits last?

A. Appointments last between 1 and 2 hours.

Q. What should I bring with me on my first visit?

  • A list of all medications you are currently or have recently taken
  • All inhalers, spacers, and/or peak flow meters
  • Videos (VHS only - we do not have a DVD player) for children who are to be skin tested. (We also have a variety of Disney and live animal nature videos.)
  • Insurance cards
  • Recent CT scans of sinuses
  • Pertinent Medical Records
  • Previous allergy evaluation if within the past 2 years

** Please note - Food/ Beverages are NOT allowed in the office except for bottles of formula for infants.
