Our site's registration form requires users to give us contact information (like their name, email, and postal address), and demographic information (like their zip code or age), and personal information (like insurance provider and social security number).

Contact information from the registration forms is used to get in touch with the patients when necessary and to register the patient in our system.

Any information that you send us may be sent to your insurance provider to verify insurance. We do not use share your medical or demographic information with any other parties. We do not even use this information anonymously with other companies (i.e. drug companies). All patient information adheres to the HIPAA Privacy and Security guidelines.
